The shallowest gas fireplace that comes to mind that still looks looks a little like a fireplace is the Heat & Glo Soho: http://www.heatnglo.com/Products/Soho-Gas-Fireplace.aspx?page=Specifications It's only 11 1/2" deep. The Solaris is shallower at 5 1/2" deep
They are quite expensive, between 10k and up. They should not be considered heating elements, they will heat the room they are in through radiant heat (they are sealed).

I do not know for sure as I was not involved in the design of this space, but it looks like soapstone. Soapstone is extremely heat resistant and stain resistant, so it is a good choice and is often used in fireplace surrounds. Just a thought.

I have been told to have shelf inbetween to protect TV from Heat. Is it necessary?
something to divert the heat from the fireplace is best and a mantel will help do that. The other option is to recess your TV into n alcove. Your TV manufacturer will have specifications on that.
