That looks like the George pendant from Rejuvenation. The shade may be custom, show them the picture.
The tile is from Ann Sacks, it is part of the Medina Collection. The pendants are vintage mercury glass shades that were mounted on pendants from Rejuvenation House Parts. They often have vintage lampshades that they will gladly mount to one of their pendant
