I'm not sure about the exact one pictured, but at Architectural Antiques we have several slate mantles similar to this one. Check out our website, this link will take you to the mantels. I'm not sure if you have a local architectural cast stone fabricator but if you do they could design this exact mantle for you. In the Dallas area, I use North American Stone and they have a similar design in the link below. Possibly a more costly alternative but you will be able to get exactly what you want.
