What kind of tree is this?

Hi. Thanks for the question. The tree is a Physocarpus (nine bark) with the cultivar name of Diablo.

Jay, I have never seen ninebark in tree form before. Is it shaped by you this way?

The diablo is a larger ninebark, so it can be grown by training a central leader like this. It's not grafted. Just a bit of trimming every now and then is all it takes. Perhaps your nursery or plant supplier can find you one. They are awesome.
