I have to many focal points in my great room, the fireplace, tv and a large window over looking my property. I want to put the tv above the gas fireplace. Is there any rules for the height vs the distance from the tv, or the size of the tv? Thanks for any help you can give.

It truly depends on the distance from the tv and if you will be hurting your neck each time you sit and watch it. Mounting it will help with the focal point problem as long as it does not seem like the tv is on the ceiling.
Care to share a pic of the room???

Make a mock up of a cardboard cut out and tape above the fireplace and then sit on the couch and see if it hurts your neck then move it up or down as needed

Yes there are minimums based on projection as heat flows up and out. Here is the manual for Kingsman. Look on page 26. Other manufacturers may have different guidelines. Just measure your projection on your tv, and see that applicable minimum distances.
