Try an online search for "library ladders" and it will lead you to many good sources for similar ladders.

This ladder looks easy to make, though it's not clear from this photo what hardware is used to keep the ladder attached to the shelf for safety. Hire a carpenter if you don't feel up to the task or have the tools. This should only cost $30 in wood and if you hire a low cost carpenter off craigslist for $25/hr, he can make this in 2 hours.
