Could you please tell me exactly what type of stone was used on the fireplace wall? Thank you.... I can not say exactly what stone is on the fireplace, but it has the feel of Azurastone's Grigio Vanadeco.... I'm installing a very similar fireplace with a band of honed onyx from floor to ceiling. I'm considering installing a floating mantle. I'm afraid that it would ruin the lines...but I do like having a mantle. What do... If you like the appearance of the fireplace floated up off the floor like it is in the photo above, a mantel shelf will often end up being so high off the floor so as to look awkward.
The negative space (nothingness) around the fireplace is the most important part of the... composition.

The most important reference in this case is a rare tome entitled "Common Sense and Good Judgement", which is hard to find anywhere.
Kidding aside, there are only restrictions regarding clearances... to combustible materials - that is, things that can catch fire - and those can be found in the Residential Building code for masonry fireplaces, or in the installation manuals for manufactured fireplaces.
The area immediately around a good gas fireplace gets up to 200-300 degrees Fahrenheit, and... combustible materials are allowed in an area that will still get from 150-200 degrees, which is too hot for a TV.
