Liner: The liner should not be plastic, it should be a permeable landscape fabric. For the fabric to work properly, the area needs to be excavated 2-3" below the finished grade. This allows a thick enough layer of material on top of the liner to limit weeds. Weeds: There should be no irrigation run off or overspray on the hardscape area to reduce the chance of weeds. Usually if gravel is installed correctly there are no weeds. Movement of gravel can happen, but to reduce the movement, utilize the technique above and excavate down 2-3" and add a fabric, then gravel and keep the gravel about 1/4" below finished grade. Another way to keep gravel from moving around is to utilize a jagged gravel instead of a smooth variety. Crushed gravel locks itself into place to reduce movement. Additionally placing gravel on a thin layer of sand also helps bind it together. Lastly leaves and mess: This is a very personal decision. Gravel can be clean and zen like or rustic and woodland and if either of those scenarios require cleaning to maintain, a light fan rake should do the trick.
