The in-wall speakers and entire A/V system was specified and installed by the A/V consultant, Interior Technologies, Inc. They're based in El Segundo and should be able to answer your questions
Recess the speakers for the theater system into the walls - no reason to have black pimple sticking out on the walls. Totally unnecessary.
Recess them. Let's not forget the speakers that are part of our audio visual systems. Recessing these items will keep them less obtrusive and less pimple like.

sure. the firebox insert is a linear burner system from Sparks Fire. this system allows you to do a site built firebox that you place the burner in. so the firebox is not an entire insert, if that makes sense. they certainly make firebox inserts if you do not want the hassle of providing a masonry
