Hello gbmom! This tile looks like a product by CERIM ITALIA. If you are local to New England, I believe Smethurst Tile in Burlington carries it under as "New Hampshire" in their New England Slate collection. It is a High Definition Porcelain tile though, not actual stone. Good Luck with your project
