I'm hoping to find a way to cover them but still leave access. The audio and video wiring will be run in the conduit (inside the wall) but come in/out through these holes to get to the TV on one end and the Bluray and Apple TV on the other. The whole idea was to hide the wires inside the wall as much as possible - I'd just like to make the holes as attractive as possible with some sort of cover or something...

Just paint them the same color as the wall.

A desk grommet might work. These could be painted by first spraying with a plastic paint, then painting to match the wall.
