Extending Hardwood Floors - How do you tie old and new together?

We have oak flooring in our entry and kitchen and would like to extend it into the family and dining rooms. From what I've read, it doesn't sound very difficult to match the old and new colorwise if you do a complete refinish. However, it seems a lot harder to tie the old and new planks together. To avoid having a line, do you cut out several of the old planks so you can slide in longer ones? Or do I have to have a border between them?
