I don't understand the significance of the 2 hole, 4" apart unless you are talking about the valves that control hot and cold water.

Anyway, if all you want is a sprayer, you don't need a shower. You can buy a rough-in valve with a fitting that accepts a shower and a diverter spout. Connect a hose to the shower fitting and a hand spray at the end of the hose. Dog bath solved. Whole thing out to be under $200.00.

If what you want is a rustic shower, how about a galvanized outdoor shower with side spray? Ought to be plenty rustic. Try Signature Hardware.

Our you can make yyour own with an outdoor valve and some galvanized pipe. Thread a shower head onto the far end of the pipe and you have a shower.

If you don't want to make it yourself, try the K-7258-RP galvanized industrial shower from Kohler. It's wall mounted so it can be set over your tub and fed from the same hot and cold water pipes.

Yes, I mean the tub has 2 holes 4” apart on center. My plumber said I needed to find a set up that fit the existing holes.

OK, 4" is a standard. You should have no problem. But, my question is why are you doing this. Can't your plumber help out? Selecting these kinds of fitting can get very involved for a layman.
