It's a Geo Decco 8"x8" white with black stripe tile that I purchased from Ceramic Tile + Design in San Francisco - Unfortunately, I didn't write the name of the company down and the code on the invoice is hard to tell the name of the manufacturer. http://ceramictiledesign.net/collections/

Can you please tell me what tub, tub fixtures and glass surround you used?

The tub was Kohler (K-876), shower fixtures were Grohe (Eurostyle Cosmopolitan collection) and the glass was just a barn style slider. Any shower glass installer should be able to do it.
