I would hang the curtain rod half way between the trim and the ceiling. You can mock it up with blue tape.

as high as you can. Are they standard panels, off the rack? You may not be able to hang the rod right under the ceiling, but higher rather than lower will be an illusion of height and dray your eye up... always a good thing, to me.

Doing custom so any height I want. And yes doing patio door too in kitchen where ceiling is lower. Wondered if that matters

Yes, as high as you can go. You might consider a traverse rod for your patio doors so that you can pull drapery fully over to the right side and be out of the way of your door handle/light switch.

Curtains go to the ceiling in the flat ceilinged space and match in the adjacent space.

All good advice, but since you are having them custom made, your curtain fabricator is the best source. He or she is in the room and has the advantage of seeing everything in person. I always take the advice of my very experienced and knowledgeable workroom. Make sure you tell him/her what your goals are, what style you prefer, what type of hardware you want, and what fabric you are considering. All these factors will determine where and how to mount the curtains.
