On item 3, I would add that you should have a clear written agreement regarding the approval of additional work that is not included in the contractor's budget (or included as an estimate only) and approval of the additional cost (for items over, say, 10%). If you're going to keep on top of the budget, you need to know what that extra work is and what it is costing.
