can you please tell me where it's from? or was it custom made?

A: Well...unfortunately this is an antique that is actually a modified half of what was once a larger table.  An ex-wife got the other half.  LOL

What are the dimensions of the fireplace surround? What is the size of the

A: It was a simple cinderblock constructed addition with a smooth skim coat.  There was not a fireplace originally.  The opening is about 34" SQ., 22"D.  The ceilings were only 8'.  Taking them down gave the look of additional height.  Note to builder...be sure to size the chimney flu for the size of the box to get the proper draw.
