I would also like to know where to find the fireplace.

A quality artisan concrete shop can fabricate a fireplace like this for you. Almost any 3D shape is now possible. You can also have almost any color. The complexity of design and the cost will be relative to each other.
Not all concrete is created equal, so make sure you find an experienced fabricator with a solid concrete mix design (pun intended).

I would bet that it's a shell fabricated of a solid surface material such as Caesarstone.

m.b. and bepsf: thanks for responding. Either material would work, but I think those sharp edges require solid surface.

Who says that a fireplace has to be square or rectangular? Cast stone, concrete and other engineered materials allow designers to create any number of shapes to tickle their clients’ fancy. The trapezoidal shape of this fireplace is an unexpected but nice juxtaposition in this quiet room.

Essentials: If there's glass on front of your fireplace, you'll probably have to clean it at least once a year. Parks suggests cleaning with a water-and-vinegar combination or a specialized fireplace cleaning solution (avoid cleaners containing ammonia). If the glass is collecting soot, your fireplace should be adjusted to burn more cleanly.
