What is this kind of trim called? Is it a custom design or available commercially? Thanks much for any answers.

I would like to know this too. Should I have my trim planed so it sits closer to the wall like this?

This is a 5 1/2" v-groove sheet goods product from ITC Millwork in Charlotte, NC. It is only 5/8" thick. However use 3/4" boards will just sit off the wall a bit more and you can fir your 1x base & top rail out to either sit flush or extend 1/8" like in this picture. I wouldn't think you would need to plane the trim at all.
