    It looks like a fixture called "Confuzion" that Lightning Bug Lighting (LBL) used to make but they stopped making in January. There may still be some out there or you may be able to contact them and see if they can still make - there were several different sizes as I had one bigger than this.

    It looks like a Harco Loor brand of lightening, which is manufactured in the Netherlands but can be purchased in the US.
