How do I write a Materia Medica page?

I just started my class in basic herbalism today and as part of my homework I have to write a Materia medica page on chamomile. Does anyone know how I would go about doing this? Is there a specific format used? Our instructor didn't tell us HOW to go about doing it.

Latin name, parts used, constitutuents, actions, indications, dosage. Could have drug reactions, contraindications, side effects and receipes as well.
