What's wrong with my tree?

Hi there, I'm not a frequent poster to the forum but am a frequent visitor.

I have a Bearss Lime tree, with its tallest branch is only about 4' tall. I planted it Nov '10 and we had a BIG freeze that winter! However it survived. However it looks very sad nowadays. I fertilized last month (here in AZ it's recommended to fertilize citrus Feb, May & Sept) with AZ Best's fertilizer. It has quite a few blossoms and a few teeny limes, but hardly any leaves. I'm wondering if I need to do something for it beside the plant food so it'll leaf up?

Here's a couple of pictures:

BTW, I have coffee grounds on the bottom of the tree.
