1) transplant shock. inevitable, but the plant often recovers.

2) failure to match plant with environment (wrong soil, wrong light conditions)

3) failure to break up the root ball

are the obvious ones... if you could share a bit more about where plant came from, where plant is now, how you prepared the hole, that sort of thing?
