Large Maple Dying

Please help - we have a large maple in the center of the lawn. It seems to be stunted & now dying. I noticed the white cocoons on the leaves but don"t know how to treat it. My neighbors is perfect so it can"t be the enviroment. Please advise. Thank you, Debbie.

If anything the scale insects may have been attracted by the tree being in distress. What, specifically constitutes "stunted and dieing" in this instance?

There are no where near the amount of leaves that were on the tree usually and the leaves don"t seem to be fully developed. It is a beautiful tree & I want to save it. The trunk is about 2 feet in diameter so it is not a baby. Thank you for your help.
