Hard to tell from the shot if it's two pods growing from the same bud end or a small pepper sprouting from the side of the larger one?

If it's the latter then I'd say pretty rare, I've only seen small sprouts inside (not outside) larger pods. If it's the former then it's not rare but it is uncommon. I usually see it happen two or maybe three times a season out of 20 to 30 plants.

Its pretty rare but does it does happen occasionally. The seeds are still viable if you wish to save. However, I do not believe that they will continue to produce pods like that only. This is just one of those fluke things that happens every season. Last year, I had a Thai chili that did that.

This year, I have a Jumbo Cayenne that has a twin too. It is just a extra pepper to me and he will bite it with his brother when the time comes.
