Every now and then, I find a tomato that looks great on the outside, but is black and nasty on the inside. What is this and is there anything I can do about it?

There are a couple of reasons that I can think of as to why this happens.

First, there is a form of BER where there are NO external symptoms but the dead tissue is black inside.

Second, small worms can bore holes and allow the normal bacteria and fungi found in the environment to get inside and initiate a rotting process. Same thing happens if there's any break in the skin due to anything.
