It is a Kohler K-876-0 Bellwether Collection 60" Three Wall Alcove Cast Iron Soaking Bath Tub with Integral Apron and Right Hand Drain: White. We also used a KOHLER K-7259-CP Clearflo Brass Toe Tap Bath Drain in Polished Chrome which comes separately. Thanks for your interest, Alison

Off the top of my head the knobs are from Restoration Hardware. I'll go through my files and let you know where I sourced the lights from.

I'm pretty sure I got the sconces at Lamps Plus, but don't see them in their current catalog. The closest I could find are these from Fergusons http://shop.ferguson.com/product/newport-brass-NB19-51SC-polished-chrome-654309

It was custom made by Stephens & Sons in Loomis, California. Its a simple door based on a shaker style but with a little more detail in the recess. I'm sure that a cabinet maker local to you could put together something similar based on your dimensions.
