Mostly design. If you are a really big cook who splashes a lot then it might make the wall easier to clean (don't use high - style textured or 3D tiles in this case). Personally I prefer just a 4" backsplash in the counter material. Note in an older home with uneven walls the backsplash can work to hide any gaps between counter and wall.
