Help! What is wrong with my Windmill Palm?

My windmill palm is approximately 6 yrs old. Today upon inspecting, I have noticed that on one side of the plant ,the roots are exposed. Also, some weird looking mushrooms have been growing around it, on the other side where the roots are not exposed. What do I need to do??
