It is by Artisan Stone Tile, part of their Accents collection (the Flourish design; see their website for vendors). It's a 3x3 tile that is - as the collection suggests - intended as an accent tile (so it isn't meshed and ends up being a tad pricey...but a lovely splurge the client was willing to make, considering the impact).
