    Hello, and thank you for your question. I don't have the exact model number for this toilet, but I'm certain that it was a Kohler product. I hope that helps in your search!

    That looks like our 'Devonshire' toilet #K-3837-0. The trip lever comes standard as chrome. If you need another finish the trip lever is product #K-9167-L. I hope this helps! http://www.us.kohler.com/us/browse/bathroom-toilets/_/N-2569

    I am sorry I do not know the sink manufacturer. It is now a Kohler sink. The faucet is Kohler Bancroft.
