The most important thing you omitted to tell us is....where exactly do you live? Best to put your zone, country, or even state if you are USA after your name via your profile page, that will help with any further questions you have when you get your plants :)

Gill is correct that adding a location (Country/State) would assist readers.

If you reside within the U.S.. You do not need an Import Permit for any order of trees unless it is more than 12 trees.

She will get a Phytosanitary Certificate which is a must and knows of requirements of the U.S.. Many countries such as mine, you are required to have an Import Permit. Then this is where the Added Declaration is required for her to fulfill on the Phytosanitary/Quarantine Certificate by their Agriculture Department.

She needs to place this statement on her website to cover herself as the responsibility lies with her customers to secure this information.
