The sink is a Brockway Kohler sink and the faucet would also be made by Kohler. Here's a similar faucet by Kohler called Cannock.

I disagree. The design looks like the Chicago Faucets 225-ABCP Chrome Wall Mounted Utility / Service Faucet with Lever Handles. Two clues lead me to this conclusion: its body is shiny chrome, whereas the Kohler Cannock has a rough plate body where the water mixes after entering from the supply lines and before entering the faucet; and the "T" where the handles intersect the vertical body of the faucet is parallel with the upper water supply hole in the sink. The Kohler faucet handles intersect the body halfway between the two water supply holes in the sink.

Correction! I checked with Chicago Faucets. The correct faucet to replace the Cannock is the 225-261 ABCP. This is the one shown above. It has flanges on the inside of the sink, which the 225 ABCP does not have.

Wow, thanks for the info Megandean! I was going to go with the Chicago Faucet anyway because I like the metal the body is made of better.
