Due ti previous neglect, let it grow. Removing it now may upset the new growth. Do it the next time you have new growth emerging.

Dendrobiums don't like when their roots are disturbed. Experienced people suggest remove old medium as accuratelly as possible. Best solution for dendrobiums to grow in inorganic substrat, like lava rock, for example.

Yes, wait til it has some strong new canes. Eventually if they are crowding on one side they may leap out and you'll have some growth outside of pot. Just before these growths produce roots or too many I'd repot into next larger size.

These plants can tolerate quite a bit of abuse. But, less disturbance for any orchid the happier they are.
