Hmm...when all the leaves on a phal start to go yellow like that chances of it recovering become slim. Hopefully we can do our best to save it.

First-would you mind telling us a few things about how you care for the phal?

1. How often do you water? How do you water (running water through the pot, soaking the pot, etc.)? Have you gotten any water in the crown or in between the leaf joints (where the leaf connects to the stem)?

2. It looks like it's potted in bark-is this right? Does the pot have drainage?

3. When did it start going yellow?

4. How much light does it receive?

5. Has it been fertilized recently?

Would you mind taking a look at the root system and taking a picture of it?

Hi! I know i ask so many questions! Hahah! Just want to make sure everything about my plants is alright!
1) Once a week! I water it then wait for bark to be dry. I water with about half of cup (maybe little bit more) tap water. I was trying to be careful to only water the soil!
2) Yeah it is bark. But my other healthy orchids have more bark in it. Yes it has drainage hole!
3) Maybe about a month! But it wasn't that yellow until past week. It was bright green and i thought it was just originally that color.
4) About 4 hours a day maybe even more
5) Nope! I didn't fertilize my plants! To be honest i dont know how to find fertilizer and how to apply it. I check local stores and nurseries but couldn't find much! I will try harder!

I agree with Lily. For now, the best thing to do would be to unpot it and clean off the root system. Hopefully you will have at least one root left over on the plant.

I am so new to this I now know how to communicate. By communicate I mean to ask you a question and be able to get an answer. I'm 63 new to this new technology and you to orchids a little girl by the name of Miss Orchid girl on Orchid nature.com got me addicted to orchids when my daughter-in-law gave me one for Mother's Day last year and I had to learn how to take care of it. I now have rescued 9 orchids different kinds and I live in Florida. I have some on my side porch inbark clay pots and little ones on my windowsill that are phalaenopsis. I do have on my windowsill this great big floppy eared as I call it that has now produced another leaf but the floppy ears are still the same how long will it take if ever for the leaves to perk up?

Desen, it looks like there's actual soil in your pots, under the bark? That is most likely what caused the death of your orchid. The roots suffocated in the dirt. No soil for orchids. Only bark or loosely packed moss, in a pot full of holes. They need lots of air.
