New site finished floor problems

Hey guys,

I have a different post going about a "dent" in the floor, but found some other concerns and would love a sanity check....

1) after my old floors were removed, i saw that a chunk of my sub floor was breaking off.

I assumed it would get taken care of. However, it looks like the broken piece was removed and they just put the new flooring over it. Im guessing the hole is 12x3 inches:

Is this acceptable? Im just worried this will bite me in the future when the planks are thinner.... like it might crack under pressure without a subfloor

2) there are 10 or so spots on the floor that either have dirt stuck under the poly... or they are small chunks of poly. They are pretty small, the size of an ant or so. But if u step on them with bare feet you feel it. Just wondering if I should try and get my contractor to fix it (not sure how), or try to fix it myself (not sure how, other then chizeling it away and hope there isnt a big dent)

Thanks in advance.
