Replacing wall vent for bathroom vent.

Our bathroom fan vents out the wall. The flap on the damper is stuck open and we got a bird's nest in there this summer and I am also worried that cold air will come in over the winter. The vent is very old so I don't mind replacing it. How hard is it to replace just the vent part that sticks out on the wall? Can it be done from the outside without removing everything?
