I am having a hard time finding what type of tree I have. It's the only tree of its kind in our subdivision. I've tried quite a few "tree identification" sites and can't seem to find it.

It's deciduous. Loses leaves in Dec/Jan and they're back by middle March.

It's only about 15 feet high. Grows slow. It doesn't look as healthy as I think it could so I would like to know what it is so I can't treat it right.
