The roots look okay, but the yellowing from the center suggests crown rot, which is really not reversible. You'll have to wait and see if it will put out any new leaves. I can't really tell: what is the dark blob in the center of the roots? Is looks like a chunk of potting soil?

Thank you for responding. Yes, the dark blob is some type of potting soil but the majority of the medium was bark. I was surprised to see the soil when I removed the orchid from the pot. I have new orchid mix and will remove the soil before replanting.

At this point, nothing commercial growers do surprises me ... from potting in peaty soil to including instructions to put ice on the orchids. So dumb!!
