Help identifying this species and reviving it.

Hi, I was given this tree some time ago and it's progressively gotten worse, I've been unable to pinpoint a specific species of "Ficus" and am hoping someone can help with this and provide some solid pointers regarding nutrition and methods for reviving this tree. I believe it's approximately 20 years old.
