Dwarf Banana indoor plant sick?

My dwarf banana indoor plant was grown from seeds from october 2016. it was always indoors. In the past month i've noticed that there is rotting in its leaves, spots and decay. Also there is white residue on the soil, im not sure what infection is going on. Anyone knows what i can do to help it? Here are some pics

Brown on the leaves can be normal.

The yellow spotty stuff may be spider mite damage.

Stuff on soil looks like fungus, it's hard to see. Either repot with all new media, or you can try scooping out the affected area and use a fungicide. I use apple cider vinegar, 1 teaspoon per 16 ounces of water.
