What height does the shower pipe come out of the wall from the floor? Is that height considered during design as from the "floor" floor or from the shower floor?

The height can vary for each client. In this photo, the showerhead is mounted at 7 feet above the finished floor of the bathroom (not the shower floor). The pan for the shower is formed after the plumbing is roughed-in, so you typically specify the height based on the main floor level. For taller clients, I will raise the height.
