Lime Tree, dead leader

Hi guys

Not sure what the best way to deal with this is.

Have a 2-3 year old Tahitian Lime in pot.

The leader seems to be dead and woody, about the top 1/3 of the tree, however the leader doesn't really have any horizontal branches in this part to speak of

The tree has just had a new flush of growth, everywhere but this top section. Rest of the tree seems fine.

Do I need to prune it back and try and train a new shoot upwards ?

The next highest horizontal branch is too stiff and has that woody coating forming to bend up.

It did get slammed by leaf miner and then aphids straight after so that surely didn't help things.

What causes this and what's the best thing to do ?
