It is a Spanish look porcelain tile from a location tile shop in Charlotte, NC - Queen City Stone. Cement Tile Shop, Tile Collection or Cle Tile would all be good resources outside of Charlotte.

The floor tile looks like Bedrosians Palazzo Florentina in Vintage Gray (?).

It comes in 12"x12" or in 12"x24" (which actually consists of eight 6"x6" adjoined tiles), which I think is what's depicted here.
