Tile or Stone flooring

Can you please tell me the brand and tile that you have used for flooring of the area? and where can I buy them from?

My recollection (the project was done over 10 years ago) is that this is Black African Slate with the band being black dolomite tile. I'm not sure who stocks the slate these days. The dolomite was from American Stone Products in Richmond, CA.
