There will be a small portion of the exposed walls left above the shower surround, so the general practice usually is to tile up to the ceiling.

As the floor goes, you want to size the tile to be proportional to the size of the bathroom... In other words, larger bathrooms can use larger tiles, 12- 24" or larger, and smaller bathrooms typically will use smaller tiles, 4-12" unless 1x1" mosaic floor tile is used.

The idea is to have a visually appealing and balanced look.

In regard to backsplash, the general practice is to install one to prevent water damage to the wall around the vanity and behind the cabinet down the road.
