Fan inside the shower

It looks like there is a fan inside the shower. Does this create a draft and make it chilly to shower, or is it a special fan?

    Also was curious to know if the ceiling is also covered in the solid surface material?

    The fan is actually outside the shower, this is a shadow that is showing the the shower. The ceiling is simply painted.
