How does the mounting installation work?

How is the hardware affixed to the tile wall? Because of the weight I assume a tension bar system would not be adequate.. does it bolt in?
Also..it states.. out of plumb installation adjustability: No.. does that mean if the walls are out of plumb it will present installation issues?
