Chair is from an antique store in Nantucket, MA. I don't think it's there anymore but I have seen these chairs around. Probably can find one online under blue antique chair. Sorry not to be of more help!

It is! Our dogs love it. We did custom foam beds and painted the walls a dark color, then hung photos of the dogs inside. Our friends love to stick their heads in and look at all the photos.

Is there a larger opening somewhere or crawl in to clean?

No larger opening but if you pull the dog beds in it's easy to climb in to clean. Our electrician fit inside to put in can lights, the carpenter to put baseboards, painter to do the "walls", and myself hanging photos. Plenty of space ;-)
