Most of the time framed mirrors come with solid hangers at each end. Most people won't try and get the 2 wall anchors in the exact right spot to accommodate so they run picture wire between the two solid hangers and set one anchor in the center. If the wire is not quite right enough, the weight of the mirror will cause the mirror to tilt forward do to the weight. Just about guarantee that is what you ate seeing in this photo.
